What Queries Does The New York State Traveler Health Form Ask?

The New York State Traveler Health Form is a mandatory document for all passengers entering the state, including their names, phone numbers, and email addresses. This form is required for all individuals coming into New York from non-contiguous states, US territories, or any other country, regardless of whether they are a New York resident. If a traveler has left New York for less than 24 hours or is coming to New York from a contiguous state, they must complete the NYS Travel Form.

Compliance with CDC guidance for international travel is required in New York State for all travelers arriving or passing through New York from other countries. Hotels, motels, inns, and other short-term rentals must confirm that guests arriving from any location other than New York State or a state contiguous with New York are required to have appropriate accommodations for quarantine.

If a traveler answers yes to the questions, complete the rest of the form. If a traveler answers yes to the questions, see a healthcare provider before returning to work.

NYSAR has revised the “COVID-19 Health Screening and Travel Advisory Questionnaire” to reflect the change from a 14-day quarantine to a 10-day quarantine. If symptoms develop, travelers should immediately self-isolate and contact their health care provider or local health authority. However, they must fill out the form upon entry into New York State and take a COVID diagnostic test four days after arrival.
